Patient Information

To ensure a smooth and efficient experience, we kindly request that you bring the following items to your appointment:

• Your photo ID
• Your health insurance card (if applicable)
• A written referral from your primary care physician (if required by your insurance carrier)
• Any copay, deductible, and/or coinsurance amounts (if applicable)
• An up-to-date list of your medications

Appointment Policy

We understand that unexpected situations arise, and we are more than happy to accommodate changes to your appointment schedule. However, please note that we require at least 48 hours' notice for cancellation or rescheduling of surgical appointments and at least 48 hours' notice for cancellation or rescheduling of cosmetic appointments. Failure to do so may result in a fee.

Insurances Accepted

We accept a wide range of insurances, including Aetna, Blue Cross Blue Shield, United Healthcare, Humana, Medicare, and Tricare. Please note that it is the patient's responsibility to verify their insurance coverage and provider status with their insurance carrier.
